What do I do if I choose not to become a TrustCare member?
	My practice, Perisseia Physicians, will be open as usual and patients will be able to see Susan Eldred NP-C, and other associates. The same Managed Care contracts apply to as before and there is no need to change PCP’s on your card at this time. As you receive notification that I am no longer on your Plan, you may change your PCP.  If you choose to seek medical care elsewhere, my staff will be happy to assist you in the transition with regard to finding another provider, medical records, etc.
