What will the fee be?
	The annual fees for TrustCare are*: Adult 50 and over:                $2000/year ---- $550/qtr if paid quarterly Adult 40-49:                         $1800/year ---- $500/qtr if paid quarterly Adult 30-39:                         $1500/year ----$425/qtr if paid quarterly Adult up to age 29:               $1000/year ---- $300/qtr if paid quarterly Adult couples will receive $200 discount on second adult member Dependent child/adolescent of Adult Member $600/yr ($500/year for each additional child) ---- $200/quarter if paid quarterly ($175/qtr for additional children paid quarterly) *these rates apply to existing patients of Perisseia Physicians as of April 1, 2008.
