Initial visit fee (comprehensive physical, health risk assessment, body fat/muscle analysis, blood work, urinalysis, EKG) - $250.00 - due at time of initial visit

Twelve weekly visits for weigh in, progress report, vitamin/metabolism-enhancer injection $75.00 per visit payable in two installments of $450.00 at visit # 1 and $450.00 at visit # 7 - Patients who pay in advance for entire twelve weeks will receive discount - ($800.00 payment for entire 12 weeks payable at visit # 1).

Enhanced program with pre-prepared meals sent to home and additional weekly phone follow up with lifestyle counselor - approximately $280.00 per month additional depending on meals ordered.
